The Tillsonburg Medical Centre is once again offering the option of the Annual Fee Program for Extended Coverage, as this choice is preferred by many patients. As you are aware, the Ontario Health Insurance Plan pays for most medical services. However, some medical procedures and most administrative services are not insured under Ontario’s provincial health care system. Therefore, payment for these is responsibility of either the patient or the agency requesting the service.
Attached is a list of individual pay per service fees.The option to enroll in the Annual Fee program can amount to substantial savings for patients as unexpected needs arise over the year. If you would like to enroll in this program, complete the enclosed form and return it to our office. Please update any family and demographic information.
